In addition, there is a security deposit fee - a one month fee for the program the child is registered in.
Information on the registration form must be completed in full including:
- Registration Package
- Four recent passport-size photos of your child
- Confidentiality Agreement
- Photo, Field Trip, and Social Media Consent Form
- Medical Care Form
- Copy of Immunization Record(s)
- Birth Certificate
- Parent Handbook Acknowledgement Form (signature required)
